CASE SCENARIO

A 22-year old male presents to the Emergency Department for evaluation of testicular pain. The patient states he awoke from sleep approximately 30 mins ago with acute onset right testicular pain. He has had several episodes of vomiting. The pain is constant and severe. On examination the patient has swelling and tenderness to palpation of the right scrotum. You are unable to elicit a cremasteric reflex on the right side. You suspect that the patient has testicular torsion.
