A 77 year old male patient presents to the Emergency Department in acute respiratory distress. He has a history of hypertension, coronary artery disease, and systolic heart dysfunction with an ejection fraction of 35%. On exam he has rales throughout his lung fields, jugular venous distention, and bilateral lower extremity edema. The patient is in respiratory distress.
